Hazardous Walking Site Review

This form is used to request an evaluation by the CCDS Transportation Department to determine whether a walk zone to a bus stop or school should be classified as hazardous.

Bus stops, walk zones, and hazardous areas are regulated by FS 1006.23 and FAC 6A-3. Before submitting this form, we strongly encourage you to review these brief guidelines. You can view the form used to evaluate hazardous walking conditions here. Please note that FS 1006.23 uses the following definition for hazardous walking conditions:

1006.23 Hazardous walking conditions.—

(1) DEFINITION.—As used in this section, the term “student” means any public elementary school student whose grade level does not exceed grade 6.
